Immediate Postoperative Complications of Gastric Resection

Abstract
430 patients who underwent gastric resection in Seoul National University Hospital for either gastric neoplansm or peptic ulceration have been reviewed.
The distribution of complications were 9 cases of intraluminal hemorrhage, 14 obstruction of distal stoma, 5 functional obstruction of efferent loop, 3 dumping syndrome, 3 adhesive ileus, 2 band ileus, 3 paralytic ileus, 1 internal hernia, 3 duodenal stump leakage, 4 anastomotic leakage, 8 intraperitoneal infection with abscess formation, 6 pancreatits and 6 jaundice within the category of specific complication to gastrectomy, and 24 wound disruption, 14 pneumonia, 10 atelectasis, 7 thrombophlebitis, 4 renal failure, 2 congestive heart failure, 2 post- operative parotitis, 2 urethritis and 3 miscellaneous complications such as hepatic coma, shock and cystitis within the category of not-specific complication to gastrectomy.
As described above, the 108 patients of this series; i.e. 25.1%, have the 135 complications.
The incidence of complications according to the disease were 23.7% in carcinoma of stomach, 23.1% in gastric ulcer and 31.8% in duodenal ulcer as the highest one.
Most of the operations were carried on in the age group over 40 years, and though there was no significant difference in the incidence of the complications in different age group, the mortality rate was significantly higher in the age group over 40 years than under 40.
The incidence of complications according to the technic of operation was slightly higher in antecolic anastomosis than retroclic, and in Polya method than Hofmeister.
The incidence of complications in emergency operations was higher than that of elective operative about one and half time.
The cases of peptic ulcer perforation operated at more than 24 hours after the perforation have more than doubled incidence of complications comparing with those of before 24 hours.
